• Commitment to Participate Form (6K .RTF file) - to be returned with Official Purchase Order
• Schedule 4 (8K .RTF file) - one copy to be downloaded and completed by ALL sites joining this new Agreement, and one copy returned to Chest
• Implementation Form to be completed by sites adding or deleting implementation codes
• Modification Form to be completed by sites modifying their licence requirements
• Technical Contact Details Form for all sites to define their technical contact details (8K .RTF file).
